Insurance & Fees

I accept (and directly bill) the following health insurance plans, as an in-network Preferred Provider:

BCBS
First Choice Health

Uniform Medical Plan
United Healthcare

For others, an itemized/coded receipt (superbill) can be provided for submission to your insurance company for reimbursement (plans vary).
*Herbal medicine consultation/evaluation, formulas and supplements are not billable to insurance. These services are provided in conjunction with acupuncture only.

Self-pay fees for acupuncture, due in full at the time of service:  
Initial visit $155. / Return visits $115. (Cash, check and major credit cards accepted.)

 CANCELLATION POLICY

Your appointment time has been reserved for you. Please provide 24 hours notice for cancellation or rescheduling. Cancellations with less than 24 hours notice are subject to the full session fee. Insurance cannot be billed for missed appointments.
